Athletic Paperwork Requirements:

To participate, student athletes must: 1) Pay ASB / Athletic Fees in the bookroom. 2) Turn in a completed Registration / Medical Form - The registration form is required each year. A valid medical form - including physical is required every 24 months. Additional requirements include Accident and Health Insurance and Driver consent forms where applicable. Forms are available for download.

Important Dates: School Office closes June 26th and reopens August 11th, 2009 Just a reminder - you cannot participate in practice without doing your paperwork. You cannot participate in games without a minimum number of practices. As a reminder of how seriously this is taken - remember Arch Bishop Murphy's football season - a self reported expired physical took them out of the playoffs after a perfect season! Fall Sports |
