To participate, student athletes must:

1) Pay ASB ($25)/ Athletic Fees  (1st sport, $100; 2nd spot, $50. Capped at $150/academic year. The 3rd sport is free. 

 Free/Reduced Lunch Students, 1st sport, $25; 2nd sport, $12.50. Capped at $37.50/academic year.)The 3rd sport is Free. Pay in the bookroom. 

2) Turn in a completed Registration / Medical Form - The registration form is required each year.  A valid medical form - including physical is required every 24 months.

Additional requirements include Accident and Health Insurance, Release Forms, and Driver consent forms where applicable

Important Dates:

The main office will be open most days between 8:30am-1:30pm; but will be closed the week of July 11-15th.  Please call, 206.252.3680, with any questions.

Just a reminder - you cannot participate in practice without doing your paperwork.  You cannot participate in games without a minimum number of practices. As a reminder of how seriously this is taken - remember Arch Bishop Murphy's football season - a self reported expired physical took them out of the playoffs after a perfect season!
