Registration for 2010 Season

We are club team comprised of players from Nathan Hale, Ingraham, and University Prep.

All players need to have a Registration and Physical Form on file. (This includes the Physical Examination Form.) Other forms necessary are Nathan Hale High School Driver Consent (posted on the forms link), Player Code of Conduct, Parent Code of Conduct, signed confirmation from SPS Student Athletic Handbook, Metropolitan High School League Athletic Emergency Information Card (blue card), Concussion Information Sheet, and the 2010 Nathan Hale Lacrosse Club Registration Form.

All Non-Nathan Hale players need to purchase an ASB Card - $25. Check payable to Nathan Hale High School.

Each player is also required to be a member of US Lacrosse. The US Lacrosse number for the player is required on the Registration Form. The link to the membership registration site is https://secure.uslacrosse.org/default.cfm?fuse_action=NewMem

The cost for registration is $275.00.  This amount covers the cost of coaching, reserving the fields, referees, league costs, equipment, etc.   If you would like to request a scholarship, the scholarship application forms are posted on the forms link (from the lacrosse page of the halesports.org website).  Additional donations to help defray costs would be greatly appreciated.
